Group Health Insurance

Madison National Life Qualified HDHP (High Deductible Health Plan)

$2,500 Individual Deductible or $5,000 Family Embedded Deductible

100% in-network coverage after deductible (My First Health network)

Employer pays 50% of Employee Premium

Additional coverage for family members is available at employee expense
Health Savings Account

Employees with qualified HDHP are eligible for HSA contributions

Employer pays administrative fees for employees of E.S. Wilson Transport, Inc.

Employer currently contributes $25.00 per pay period to each employee’s HSA
Cafeteria Plan (IRC Section 125)

Employee can enter into a salary-reduction agreement with the employer. The employer then uses these funds to pay

for the employee’s benefits. This allows the employee to pay for his or her own benefits with pre-tax dollars.

Employee paid portion (for family coverage) of the E.S. Wilson Transport, Inc. Group Health Insurance premium can be

paid through the cafeteria plan with pre-tax dollars.

Some of the benefits that can be realized by employees include the following:

• Lower FICA and income tax withholding due to lower gross pay

• Ability to select those benefits most needed

• Potential qualification for the earned-income credit due to lower gross income

Qualified Benefits may include:

Health Savings Account

A Health Savings Account (HSA) is a tax-favored account set up exclusively to pay certain medical expenses of the

account owner, spouse, and dependents. Health insurance coverage must be provided under a high deductible health plan.
Eligible Individual: Only an “eligible individual” may establish, and then contribute to, an HSA.

This is someone who on the first day of any month:

1. is covered by a high-deductible health plan (HDHP)

2. is not also covered by another health plan that is not a HDHP

3. is not enrolled in Medicare (generally, under age 65)

4. and may not be claimed as a dependent on someone else’s tax return
High Deductible Health Plan: A health plan that meets certain requirements (adjusted annually for inflation) regarding

deductible and out-of-pocket expenses. The group health insurance offered through ES Wilson Transport, Inc. is a

qualified High Deductible Health Plan.
